Portal glitch?

Hey guys, so this may have already been addressed but there's a glitch in game when you go through a portal the portal in the nether disappears and drops you down in a random location... Anyone have a fix for this or is this an actual game glitch? Any help is welcome!

This has been happening for quite a while and there's nothing you can do as far as I know.
