No, just like any other animal, they will randomly spawn only in plains biome chunks. They don't have to be unexplored to be able to have horses spawn in them.

Actually, Ozzy is correct. Donkeys and horses spawn naturally, but you must breed a horse and donkey as in real life to obtain a mule.

also once you make a mule you can't breed it with a horse or donkey or another mule, they become sterile, as they would in real life

Ollie they changed where horses spawn.  They spawn randomly in the world

No they didn't. Quote from the official minecraft wiki page on horses:

They update that page whenever the slightest detail about a horse is changed.

I presume it is like when slimes were changed to spawn in swamps, they spawned in all swamps, not just newly generated ones.
